We are working to open Hayat Academy for the community here in Orlando: a school where our children learn their deen alongside a strong academic program, from teachers who know them.
Before anything else, we need to know who is out there. How many children, what ages, what grades. That is what this form is for, and it is the single most useful thing you can give us right now.
Please read this part. Adding your family to this list is not an application, and it does not commit you to anything. The school is not open yet and we cannot promise a seat or a start date. What we can promise is that you will be the first to hear as things move, and that your answers will directly shape what we build.

Last few things
What Florida will require later
You do not need any of this today. We are listing it now so nothing is a surprise when enrollment opens. Florida law requires every school, public and private, to have these on file for each child before the first day:
Florida Certification of Immunization, Form DH 680. Your doctor or the county health department completes this. It is not a form parents can fill in themselves.
School Entry Health Exam, Form DH 3040. A physical completed within twelve months before the child starts.
Proof of age. A birth certificate or passport.
Religious and medical exemptions to immunization exist under Florida law and are handled on separate state forms. If that applies to your family, just tell us below and we will walk you through it.
